Companioned with Strain.wad is a deh file which, amongst other alterations, lets the player start the game with fists only.

Initial Bullets = 0
Playing this with GZDoom, the game starts with the pistol and is then replaced with the fists, when firing.
This may be a bug or maybe it is intended.

Playing this with PrBOOM+ the fists are displayed from the onset.

With the current version of PrBoom+UM and no config file, player starts with pistol and switch to fists automatically, i.e. without firing.
However, it depends on compatibility settings. I suppose, Strain is vanilla compatible. With -complevel 2 it behaves identically to GZDoom.

From what I recall, that's just how Doom always worked with this patch. Perhaps PRBoom introduced it as a feature or bug fix.
