The only monster that EVER gives me frustration are actually imps; when placed in areas that you must rush out to in order to do something or another for a few seconds (like picking off a stronger monster), they will happily oblige to tan your hide with a more than a few fireballs.

Only time I get bugged. 'Cause I'm forced to be in that small area and the repeated "FWOOSHH" of fireball after fireball hitting me gets on my nerves.

Archvile in a map where there is nothing to hide behind. It changes him from a standard skill-based fight to an instagibbing auto-win enemy.

Out of interest, what is the "correct" time to use stealth monsters. I cannot think of a single wad where they added to the gameplay instead of annoying me.

Stealth monsters that aren't Demons annoy me.

A lot of the Doom 2 cast can be misused quite easily by a cruel or incompetant mapper, but the monster that can perhaps most easily annoy me is the Baron of Hell. Rather than being a challenge most of the time, they're just bullet sponges. Deploy a few of them and then, no matter what weapon you've supplied me with, I'll consider the fight needlessly long. It may as well be a 1000hp Imp for all the threat it poses most of the time.

There's a few ways to annoy me with custom monsters and I think the Diabolist (particularly as used in ZPack's E2 boss map) covers all of them: Highly damaging attacks that can't be dodged (hitscans, or the burning fire in this case); highly damaging seeking attacks that are very difficult to dodge; low pain chance; surprisingly high health. As a modder I'm also annoyed when people give human-sized enemies tons of health but no real mass. If something is blown across the room by my SSG shot, I'd rather it has also died, rather than me having to run after it and get it again.

Sadness, because I'm quite disappointed the healer imp isn't in this thread yet... I don't think it's fair to include custom monsters at all if it's not one that's regularly featured in multiple maps and mods.

Cyberdemons... when you're expected to fight them with a rocket launcher. Like, of all the weapons you have you're expected to use the one weapon that they have a special resistance against??? One of my autoloads makes them only immune to their own rocket explosions so I can finally shut that part of my brain up when I play.

On a not completely petty and neurotic note: I tend to stop playing a map at the first archvile encounter. Especially the first caged archvile encounter where it's just acting as a turret or you're suddenly locked in a little room with it. Or really I just hate any monster in a constricted "trap" situation where it's obvious the mapper doesn't trust their own design enough to let the monsters run about and the situation develop as it will (a good counterexample of the archvile I think being Map14 and Map16), but far more common is the archvile turret and the archvile cagematch.

Sometimes I find stealth monsters to be an interesting challenge, trying to aim at something by sound alone. Stealth hitscans, though, should have a pretty damn good in-game reason.
