It's when he accident'y hurts you, and he's trying to kill himself. You can reproduce the same result on any map if he hurts you slightly, or you even hurt yourself with a rocket-launcher, he'll kill you first, then himself.

I just gave it a try, and I got the same result. How odd... I took an automap screenshot of this strange phenomenon.

The area circled in green is the Cyberdemon and the rockets. It looks to me like the Cyberdemon is attempting to target the enemies behind that closest wall... but... the enemies behind the wall are almost 90 degrees from where the rockets are facing...

This is probably a vexing bug, but it's funny enough to post.

I loaded map 01 of Doom II on Ultra violence. I summoned a friendly cyberdemon at the beginning of the level, I went through the level as normal and spawned a few more friendly things along the way. Then I heard the cyberdemon shooting rockets at something, so I went back to the first room and I saw this:

He was shooting at the wall, and the rockets were floating in midair as if they had 0 speed. I walked into one and killed myself.

Then, It happened again under similar circumstances. I took the screenshot.

I can reproduce this, but I haven't the slightest clue what causes it.
