Suppress episode select for Duke expansions

When starting a game with DC, NW or Vaca Raze still displays an episode select with LA Meltdown as first episode (and Lunar Apocalypse as the second for DC)
It would make sense here to skip directly to the expansion episode just as Raze already does with Cryptic Passage for Blood and the expansions for Shadow Warrior. Vaca has a separate deathmatch episode but since Raze doesn't officially support multiplayer it IMO makes more sense to disregard that (and those levels can still be accessed through the levelstart command in console) - this would be the same behavior as in CP as well
