What about "Bad Karma"? Each weapon you fire takes a certain amount of health away (until you reach 1) so you try to do the most damage per shot if possible?
How about "Countdown"? Each map starts with a certain about of seconds and starts to count down. When it eaches 0, you die. Everytime you kill a monster, you gain back a certain amount of seconds (depending on monster).
Or "Fist of Fury"? You start off with nothing but a bersker pack. There are no ammo, health, armor, or any other items in the map. Each time you punch someone, you get back some amount of health.

Act of God: Every so often God will raiin death on a random part of the map in the form of falling comets, floods, swarms of locusts etc.
