It has done that occasionally to me before, trying again usually works. (I just tried it in the new 4.0 and works fine)

Speaking of 4.0, I guess that means the bug reports will start coming in...

That bug was present on an old GZDoom version years ago, I had to change all the doors to fix it. Works fine on modern versions, though.

I think that's a leftover from old GZDoom versions, when slots only went up to 4 by default.

No, I never think it's non-sense, it's more like too many things unexplained and leave for players to think. The only problem with HL story is Valve was let it stuck on cliffhanger on the end of HL2EP2, but even they did, it most likely still have a lot of things never explained... :( The story was ...

Ah that's a problem. I don't know the technicals on how GzDoom handles scripts and all that, but basically if you have a game file that is associated with two wad files (serpentmus1 and 2,) then it will only work if you have both enabled, otherwise it'll give you an error.
