I'm a little late to the party, just found out about this addon. Nice!! I hate using god mode all the time so this is a wonderful alternative to spam saving or resurrecting myself (which defeats the purpose of not "cheating".)
The hud addon is nice too so I can check in-game.

I can't reproduce this -- I tried both running into the portal with lives left, and taking a few hits from the AV so that it triggers and then running into while invulnerable, and in both cases the game worked fine. Yeah, I just tested again and it worked fine. Maybe it was the gzdoom version at the time or maybe a conflict with some other mod. Sorry for wasting your time.
I do have another issue though. Sometimes I die instead of being saved. It's not something I can reliably reproduce so I don't have much hope for a fix. Maybe others can chime in if they experience the same.

There are a few attacks that completely bypass all forms of damage processing or mitigation. They can still be detected by an event handler, but that will only trigger after the target has been killed. So if you get hit by that you're just straight up dead with no chance for it to save you.
That's rare, but it shows up sometimes in scripted damage and I believe some mods add enemies that use it for "armour-piercing" attacks or similar.
