I have a few gripes with this:
1. Inconsistent sound effect quality. It feels weird to have high quality gun shots mixed in with Duke one liners that sound like they've been recorded on old WW2 microphones.
2. For some reason, the game seems a bit more grey than usual. Is that intentional?
3. Unable to pick up pipe bombs after you throw them.
4. M16 grenades are way too bouncy.
5. No SSG replacement.

1. Sounds was taken from original DN3D, and then volume up. I dont even know how to fix it
2. Mod doesn't change anything, maybe sprites makes the game more gray

5. There is SPAS-12, which replaces SSG, it's just have low replace chance. And you need to able SPAS-12 in mod menu, if you really want to

Tried playing it, got a massive HUD that took up half the screen. How do I fix this?

Check your display settings? Maybe you have a wrong resolution or another settings.

To be more precise: Options > HUD Options > Scaling Options > Status bar.
Play with the slider (in your case probably just move it to the right) and see if that does the trick.
