I wasn't the one playing when I noticed this. The person playing was doing so on the highest difficulty, tried every possible strategy and they could only win by saving regularly during the boss fight. The room is particularly cramped, Igor spams knives way too much, there's no way to reliably dodge both at the same time. So, I've noticed something about a certain boss... The Creature and Igor. The boss fight is WAY too overwhelming with no protection and pretty much demands you have the Holy Water to be able to stunlock The Creature.
My suggestion? Add a couple shelves to the room, a surgery table and/or a desk to allow us to hide from Igor's knife spam attack.
Another thing: Make sure the autosave kicks in before entering the boss room or that there's a health item during the boss, because entering a boss fight at half health is.... incredibly harsh.

Hi! Me again. Instead of pushing for a tiny map change from the creator, I can teach you a few GZDoom tricks to help you out. IDCLEVxx where xx is the map number so 04 to do The Dungeon again where you got stuck will level warp you to the beginning as a fresh start! Now if you feel the need to cheat you can use command line "Give Health" to refill your health.
Highest difficulty Blood Moon is meant to be very hard. Even on Normal Full Moon The Creature is hard for all the same reasons you list. Circle Strafing is the only way to out speed all of the projectiles and is basically the number one skill used in any doom engine game. Whats worse? Map 05 Boss Death is even harder, and thats not even talking about Dracula's excessive levels of pain.
Mike Matei on Youtube has a full Playthrough of Simon's Destiny on Normal. The Creature is no Different on Normal than he is on Blood Moon. Blood Moon only changes the Normal enemies, all the bosses remain the same.
