SOTN is filled to the brim with these kinds of things - even the Alucard Shield (that's right. a shield) is massively overpowered, if you use the Shield Rod with it. Nearly an entire minute of not only total invincibility, but also 255 damage every 1/4-second to anything that's touching it, AND IT HEALS YOU IN THE PROCESS. Dracula becomes an absolute joke with this thing. Battleships Forever's dual railguns are stupid overpowered. They completely negate the point of armor and can detooth enemies making them harmless.
And while we're at it, the holy hand grenades from Xeno's Relics. Not that I care, but they pretty much make bosses a joke, and without damaging terrain. About the only thing they can't kill is Godzilla (but he's supposed to be next to impossible to kill)

And there's the Muramasa, which requires some grinding with certain enemies but the stat increase carries over to the other saves if I remember correctly. And you can get it before the second half of the game from a weak enemy too, so if you're patient enough you can kill bosses with it with only several button pushes literally. It follows the trope Infinity +1 sword perfectly.
The gamebreakers in SOTN was rampant that they got too paranoid after it. You can notice that each newer game received nerfs each time.
Aside from the game SOTN, various units from RA2 like Kirov, Apoc and Prism tanks. And possibly the Yuri faction.

Man, do I have entries for this list. I'm going to start small and mention the Bishop B.A.D.A.S.S. from the Guncaster mod, which makes the user immune to hurt floors, greatly increases damage resistance, amplifies melee attacks to the point of instagibbing Revenants and Hell Knights, and adds a pair of arm cannons that fire weaker versions of the mod's BFG's shots.
Weaker is relative, mind you; it still fires enemy-piercing energy balls that violently explode when they hit a wall, dealing massive damage both times. Oh, and it doesn't use ammunition; it runs off of the character's mana supply which regenerates on its own over time. And its secondary fire summons an airstrike that can twoshot a Cyberdemon. The author even stated that it's deliberately overpowered.
From another, older game in another genre, Sid Meier's Alpha Centauri's expansion, Alien Crossfire, gives the factions access to String Disruptors at the very end of the tech tree. According to the lore blurb, they work by (like the name says) disrupting strings, the most fundamental particles of matter, remotely, effectively disintegrating anything they're pointed at from arbitrary distance. It's not for nothing that they are the most powerful weapon in the game, with an attack rating of thirty.
Switching genres again, I want to bring up OBS. In short, a group of nerds on a semi-obscure debating forum called Spacebattles decided to do their best to create the most overpowered faction ever, and they did pretty well, considering that OBS routinely outfits its single-seater fighters with UGB warheads. That's Universe Go Boom warheads, for those of you at home. Top that if you can.

not really a weapon, but i've been playing Final Fantasy 4 on DS lately, and the Draw Attacks augment combined with Counter augment seems like it's almost the reason they made the game this hard (Draw Attacks draws most of every attack towards the recipient, and Counter counters any physical attacks made to the user.) not to mention, it's two augments you don't need any foresight to get at all and is given to you. you never have to worry about your squishy members at all when Cecil can just tank most of everything the game throws at you. (even still, the game is hard, especially since a few encounters can kill you without you being able to react if the RNG rolls bad enough.)

The Gauss Cannon from the multiplayer in the new Doom. Insta kills any player and you literally can't miss a shot. Oh and the alt fire lets you see players through walls.
