Never had that happen. Maybe this is an issue of Mass being too low; I know the grenades throw enemies around like foam packing peanuts, as you mention in point 10:4. Shooting at the feet of Golems with a pistol will rarely cause them to fly to the ceiling.
10. Knockback ratios are way too high, especially on the explosives. They should all only be about 1/3 a the most of what it is now.

These two seem to be connected; perhaps due to Heretic/Hexen/Strife having defined item activation sounds (this is more noticeable in Hexen), you will hear a constant low "hum" which is actually the item activation sound being played repeatedly at a very high rate. Taking damage interrupts this cycle so that the item sound can actually play. This seems very likely to be the cause.
