This would require drawing a lot of new sprites, but it could probably be done from separate pieces. Like the Gibb sprites in Quake.

Given that many normal deaths in those games are kinda gory already, I'd rather see a mod that gives each bad guy a normal death and an exploding death, with a clear distinction between them. Same for Doom's chaingunner, too.

A simple minimod similar to Colourful Hell where it randomizes armor, health bonuses, armor bonuses and powerups.
You have your normal-tier stuff, then every tier up increases the max armor/health by 100, and has effects that scale up as well. Ideally, everything would be themed around the tiers of Colourful Hell.
For example, a Tier 3 Berserk Pack would increase melee damage by 5x, instead of the normal 3x melee damage, it would also heal you to 300 HP maximum (a tier 3 supercharge would ideally heal you to 400 though).
A tier 11 health bonus heals 12 hp, and can heal you up to 1200 hp.
Also, weapon sets that resemble the weapons of the Colourful Hell cast. Basically just weapons that they specifically drop of the shotgun, pistol and chaingun variety. Like a yellow zombieman will drop a 3-round-burst rifle, red zombieman drops an unmakyr, etc etc etc
A weapon mod with an arsenal (save for your fists) based on the Seven Deadly Sins, with the added gimmick that you can temporarily buff the weapons by doing certain things during gameplay that correspond to their respective sin.
Envy - Jealousy Blaster: Pistol replacement. A green sub-machine gun that has about the same rate of fire as the vanilla Doom Chaingun, but it does quite low damage. Secondary fire does a three-shot-burst that does a bit more damage. Shoots a bit faster if you're close to high-tier monsters
Greed - Con-Cash-in Gun: Shotgun replacement. A golden shotgun/cash register hybrid that shoots out coins and dollar bills that ricochet off walls. Naturally it make a "cha-ching!" noise when pumped. Secondary fire launches an explosive bag of money, similar to the secondary fire of the Flak Cannon from Unreal Tournament. Has a faster fire rate if you keep gobbling up ammunition of any kind.
Lust - Cupid's Crossbow: Super Shotgun replacement. A crossbow (duh!) that shoots three high-damage bolts at a time, akin to Heretic's Ethereal Crossbow. Secondary fire charges a single bolt that does no damage, but it instead explodes in a pink gas cloud, enemies caught in the gas cloud will be temporarily brainwashed into fighting other enemies on your behalf. Fires out more bolts and charges the secondary fire faster if you keep killing monsters fast, quenching your lust for violence. (yeah i kinda stretched with the "doing the sins buffs the weapons" system here, i know)
Sloth - Slumber Party: Chaingun replacement. A fire hose-esque weapon that sprays green slime, which stays on the ground and slows down any enemy caught in it. Secondary fire puffs out a big gas cloud that stuns anything caught within. Does more damage and has better slow-down properties if you're not moving around very much, with maximum power being achieved if you're standing perfectly still for awhile.
Gluttony - Fast Food: Rocket Launcher replacement. A grenade launcher that resembles a miniature hot dog cart, it launches a disgusting blob of meat that bounces around before exploding. Secondary fire "prepares" a hamburger that acts as a proximity mine. Does more damage if you keep consuming health items in quick succession.
Wrath - Inferno Stick: Plasma Gun replacement. A flaming pitchfork that acts as a flamethrower, naturally. Secondary fire squirts out some oil on the floor you can ignite for a trap. Increased damage and range for a short time if you just took damage.
Pride - Slayer's Monument: BFG replacement. A statue of the Doomguy himself that acts as a VERY powerful, VERY fast-firing minigun that has high recoil. Secondary fire throws the statue, turning it into an automatic turret gun. Shoots faster the longer you can go without taking damage, if you just got hurt the fire rate sinks like a stone.
Not sure how doable the whole "weapons get buffed when you indulge in the sins" mechanic would be for some of the weapons programming-wise, but I figured it'd be an interesting way to spice up the weapons compared to other weapon set mods.
On top of that, it makes the whole system a lot more flexible and scalable—like having different buff tiers based on player intensity. This kind of mechanic fits right in with the ZDoom philosophy, where gameplay conditions can directly tweak weapon behavior and how you approach each encounter.
