An install utility for Windows
I might be the only person on the planet who cares about this, but I tend to like having programs be installed into a folder and have them have shortcuts and file associations. I can do that with GZDoom, but I have to do it manually. I think having an install utility for Windows systems would be a good idea.

When I was using Windows, I used to make my own for various GUI gamedev tools using NSIS (Nullsoft Scriptable Install System). It's still available so might be worth a look and far less complicated than most of the others.

Let's be serious. Setting file associations for a Doom engine will anger most users.
Some people have multiple ports installed and others prefer to open them with an editor.
Personally I am a strong advocate of portable installations so it'll be hard to convince me to provide such a thing.

With an installer, more people would probably install GZDoom under Program Files too, and we all know the problems with that.

Not as much as before.
Used to be, GZDoom attempted to write its config to the Program Files folder. That's stopped, now.
There's still other issues - like you plain just should not be modding files inside there (i.e. using Slade or such) - because then the fuckery comes in with the virtualstore where you don't actually know what the hell is going on in your file system - just that Windows is arbitrarily deciding what to do with those files.
