I read in this big ol' fan-made encyclopedia of all things Doom that health bonuses are made by just taking a soulsphere, sitting it in a pool of nice clean water, and letting the power of the soulsphere infuse the water with a little bit of its healing power. Bottle it up, and distribute it to demons for a quick little snack when you can't get a proper soul to eat.
It's got cool explanations for the various oddities in Doom, like why armor bonuses are glowing medieval helmets made of crappy looking metal, or why the chainsaw never runs out of gas, or why you have to point the plasma gun up every time you stop firing it. Encyclopedia Daemonica, I think it was called. None of it's official, of course, but it's all very interesting nonetheless. Sadly, I don't believe they ever say anything about the presence of oxygen in Hell, either to confirm or deny it.
Oh, and it actually explains another long-time mystery, why you find medkits and other man-made things lying around even in Episode 3 when logically everything there should be hellish in origin. The answer is simple: war trophies. Souvenirs taken back to Hell by the demons to commemorate their victories over humanity. Sort of like how soldiers in WWII sometimes brought home weapons and equipment taken off defeated enemy combatants.

The author's written three other Doom-related works on there, including a story where the Gatekeeper at the end of the Plutonia Experiment, in its last moments, tells Doomguy the story of his life. Ah, one thing I forgot to mention: the fanfiction.net version lacks the pretty pictures, but has been updated with a section on the extra secret levels included with the recent console ports, No Rest For The Living, and the Master Levels. Also, some things have been changed around a bit, like the official designation of the Fear facility is a lot longer in the .pdf version. Other than that, the .pdf version is exactly the same, and has the advantage of being downloadable and stuff. But, the fanfiction.net version is the most recently updated one.

No problem. Again, while you're there, why not give his other works a read? They talk about some more interesting things, like the geography of Hell. In his fanon, Hell is composed of two big continents in an ocean of lava, with small islands scattered around and in between them. One of them is divided in half with a big impassable mountain range, and it's this continent that you visit in the games: the right half during Doom 1, and the left half during Doom 2. The left half was destroyed after you murdered Baphomet's face off, and the right half was destroyed following the events of The Plutonia Experiment when a pair of Void Spheres were detonated there.
