I actually play guitar and bass. Painkiller is NOTHING like the opening to At Dooms Gate, the chords are totally different, the drums are totally different, the bass is totally different. the only slight resemblance is its a chromatic chord progression, its a natual chord progression of the E and A chords and their octaves (except the fills), on the whole, it is an original song unlike E3M3 or the likes. Its still hotly debated what the derivative of E1m1 actually is, however, listening to No Remorse carefully and seeing a tutorial by Sonic Clang himself (who did the Classic Doom for Doom3 soundtrack, he knows a thing or two about music), theres perhaps 1 or 2 notes different (this is common by Bobby Prince for the Doom music to avoid copyrite infringement).

[EDIT: Except that this seems a lot more recent than TNT or D2 Map09 so it might be the other way around?]
