-Lives system is really picky on when it wants to work
-Grabbing an enemy that's in a group might have it end up in the possession of another npc instead of the player

-A small gripe I got with this mod is how having "Toss weapons when run out of ammo" on means quite a number of frustrating/annoying moments when picking up a new weapon and realizing it doesn't come with ammo. You either have to toss your new toy right away or hang on to a useless gun until you can find an ammo pickup, which can be tedious if the map you're playing has a thing for item scarcity.
