Smooth mouse movement?
What does it really do? I can't feel any difference with my mouse movements regardless if it's on or off.

If this is set to "yes", abrupt changes in mouse moves are compensated for to result in a fluider movement.
I highly recommend you keep it disabled. You don't want anything interfering with the way your mouse moves. You want to keep a 1-1 movement with your mouse and this will only hurt your tracking and randomize your aim. I would also highly recommend you disable mouse acceleration.

If set to true, ZDoom will interpolate the changes in view angles and view pitch (when using the mouse to look to the sides or up and down with free look) as if the player was using the keyboard. This makes mouse look more fluid and continuous.

Yeah that's the thing, I figured that was what it does but I tried swiping my mouse abruptly but still don't see it being smoothed or interpolated.

Effectiveness depends on the update rate of your mouse. Mice with low rates can produce a very unpleasant gaming experience if this is not on. This was added in particular for Logitech wireless mice from around 7-8 years ago. Those were hideous.
If you don't see a difference your mouse updates more frequently than the game's refresh rate.

And since 125Hz is pretty much the "standard" standard while being over twice the standard 60Hz refresh rate for monitors..

Yeah, it's only really relevant for those stuck with Windows 9x and its hideously low slow mouse sampling rates (but not ME as that has a smooth update rate)

The best way I can describe it is that when mouselooking with this option disabled, your view would "jitter" 2-3 pixels upward / download instead of moving smoothly. This caused quicker mouselooking, but it was difficult to perform precise fine mouselook movements.
Enabling the smooth option fixed this issue. I have since buried my Logitech mouse and replaced it with a... (looks down at mouse)... Rats!

On high sample mouse, "smoothing" a mouse would lead to the horizontal aiming drifting upward or downward (at least on UnrealEngine games)

This sounds like a problem with your mouse. I use mouselook with smooth movement disabled and have no jitter.
