I noticed #2 also... This also happens at the beginning of map03 if you look out the little window to your left right at the start... You can (barely) see a zombie there but the targetter won't put him on the radar and he won't release souls if you shoot him with the shotgun. Long story shourt, you have to have had a clear LOS on an enemy for it to release souls.
