Wads that render "resurrect" inoperable? (Wiki: "broken apart/shattered while frozen")

I have not has this happen to me in a wad before. I think I've had a damaging floor cause me to be unable to resurrect, it said something like there's nothing there to res. I thought it was after a "Player melted" but I've res'd many times after that message. I very much want to see this effect but I don't know where to look. Cold as hell?

Both of these are more likely to happen in Hexen; if the player has gibbed (and had their head pop off) or if an enemy like a Wendigo has frozen you, there won't be a player corpse left to Resurrect.
