third person doom mod idk, works great with a controller
camera moves independent of player so you can spin around the player and play ring around the rosie, it looks cool and stuff

Maybe I missed it, but is there a way to toggle between 1st and 3rd person?

You have to set the control for the "chasecam" option.

Options -----> Customize Controls -----> Chasecam

Nope, not as far as I can tell.
