That said, I have no idea.
To the best of my recollection:

- Uncapped with VSync switched on.
- Windowed mode.
- Always, especially in menus from a fresh start.

The specific set of bluetooth headphones I was using broke on me in the months since this report, though, so I'll need to look into setting up the replacement pair for any further testing.

I always play with uncapped frame rate, vsync off. Turning on vsync causes diagonal screen tearing, thanks to NVidia's ineptitude with drivers... Still, I'll try toggling it next time I'm at my desk and report back if anything changes. I always play in Windowed mode, so I'll try toggling fullscreen to see if anything changes. Everything in ZDoom (or G or Q) is affected, whether I'm playing the game or just in the menus at the title screen, or anywhere else. Nothing else on the computer is affected.

I've just had a chance to check, and there was no difference whether I played fullscreen or windowed, with or without vsync.

Still unable to test atm, but is there any difference between FMod and OpenAL in regards to the problem? It might be worth looking into.

Without any bluetooth devices to test with, this cannot be fixed. I do not personally have one - if Graf or dpJudas do not either, there's nothing we can do for this except to put it on hold for now.

I do have a Bluetooth headphone but the entire Bluetooth thing is so volatile overall that I prefer to connect it with a cable normally.

There's no way we can fix this without the proper hardware. We need a dev who actually has this.

I didn't say I cannot connect via Bluetooth, but it's not something I'd do for real use. For quick testing it should be fine. But even then I probably won't be able to do much because FMod's innards are off-limits.

@Ed: Have you tried different sound outputs for FMod (e.g. DirectSound vs. WASAPI?)

Different sound outputs seem to have no effect. Of course, since this issue occurs even with OpenAL, I'd imagine FMOD is not really at fault.
