Just after the first big outside Caco attack, inside the building there is rox ammo surrounded by acid. The platform that the rox are sitting on causes damage, as does the smaller platform right next to it.
Also, there are a lot of potential rox jump level skips. Personally I like having that as an option; makes for interesting replay and alternate approaches/strategies but can also lead to a few softlocks.
I like the new area you added in this section (dark area near the barrels), spooky.
The level name in map view is Entryway.
The secret area (The Castle Was Here), right near the end of the battle you spawn in a shit load of pinkys, this could be problematic if the player is in the center of the room as they are completely surrounded.
Last, I know this is the vanilla edition but I played with BDv21 anyway (GZDoom v4.4.2) and it worked fine other than than the trigger I mentioned before when you pickup yellow skull key. Cannot figure out why BD fails with scripts reliant on picking up keys.
