I can't recall ever seeing a wad that takes more than 20% health. Also never seen one take under 5%?

Also, anyone ever encountered lava that doesn't damage you? How about water that does damage you?

Remember: Doom originally only had 5/10/20% damage sectors. And for many years it wasn't easy to create other types.
As for the two "weird" things, of course such things exist. Just download 20-30 1994 WADs and there you go. One of them is almost certain to have improperly set up damage.

though this begs the question... What is all this for?

I like to play through wads that go against the grain in terms of screwing around with stuff that was hardcoded in vanilla.

Well technically damaging water or safe lava aren't hardcoded in vanilla - as the floor texture does not indicate the damage dealt - you could have damaging FLOOR0_1
