1) How to add custom MIDI music?

Anyways, switch to gzdoombuilder-bugfix, as DB2 is outdated and not developed anymore.
For the problems, post the wad you're making. Because from your description is really hard to tell anything.

Most of these seem to be related to either not having, or badly making, a [wiki]MAPINFO[/wiki] lump.
To try and answer one of the points, unless things have changed, you can't use an entertext on the first map in a game. The usual method to work around this is to have a very simple first map that immediately exits and moves on to the next map via a script and allows the text between the two maps to be shown.
However, as ramon.dexter says, it's quite hard to tell from such a brief description exactly where the problems lie.

You're still not giving much away. All you've said is the name of your mod and that you're adding sounds and levels - like many people on here are. i.e. you've basically said nothing useful. You're not helping us to help you.
I'll try
1) Put your music into your project file in the appropriate location (a music folder in a PK3 or just in the general namespace in a WAD) and reference it in the [wiki]MAPINFO[/wiki] entry for your map or change to it using scripting.
2) First map in a mod explained above. Subsequent maps need to be defined as being in separate clusters in [wiki]MAPINFO[/wiki] before text intermissions are seen.
3) It's either a [wiki]MAPINFO[/wiki] problem or the exit line/script in your map is set up wrong.
4) Not really sure. Could still be a [wiki]MAPINFO[/wiki] problem or some other possibility.
