You seem to have linked the same MIDI (D_READ_M) twice there.

That particular one is called "In My Mind", by me.

That is the intermission track from Memento Mori, though you probably knew that (judging from what the file is called). Composed by Mark Klem, but I don't think it has a name.

Yes, Mark Klem was a composer in the Doom community back in 1996, possibly earlier. He's still active on Facebook and YouTube.
