[Shadow Warrior] [1.8.1] Detonating Accursed Head causes hang

I've found that detonating a specific Accursed Head in the level "Killing Fields" causes my game to hang for 5-10 seconds. I noticed that my kill counter jumps from 81 to 397 when I detonate the Accursed Head, so maybe that's what's causing it? I really wish that I could just attach my save to the ...

[Shadow Warrior] [1.8.1] Gas Bomb causes game to crash

Using the Gas Bomb in Shadow Warrior causes my game to crash. I've tried resetting all options, as well as deleting everything and starting fresh, but nothing seems to help.

E3M1 has a single Phantasm (on Lightly Broiled) near the beginning at the top of the first building. It's a pretty big room, and should give you enough space to bait the Phantasm into shooting projectiles at you. Sorry, wasn't sure what other info I could provide. This still happens in 1.6.2 and also the latest dev builds. It is reproducible every time. When positioned far away from the Phantasms, they shoot a projectile skull from their scythe. They're supposed to stay still when launching this attack, but ...

Phantasms lunge forward when using their projectile attack. They're supposed to stay still when doing this.

There's a section in E6M4 (Aqueducts) that has two glitched dynamite cultists up on a ledge. They can't seem to be injured/killed, and they don't respond at all to my presence.

I'm experiencing a crash when entering a specific room on E2M4 when using the Polymost renderer.

I've attached my crash report, as well as a save file from right outside the door that causes the crash for me.

Not sure if this is vanilla behavior or not, sorry if it is.

I noticed that if you only have one napalm round left, and then pick up an "akimbo" power-up, you shoot two rounds instead of just the one.

I've attached a save file with everything set up to duplicate the issue.

Okay, I just finished the level "Toxic Waste Facility" from the Twin Dragon expansion and the exact same cutscene played. Once again, this was just a regular level exit, no Sumo boss.
