No luck with the DoomRPG Launcher, either. Setting the difficulty to adaptive on the launcher still only spawns armageddon enemies. None; this problem is happening on Doom 2's MAP01 on a new game.1. How many levels have you passed?
Initially set to 1, but the same problem happens with every setting between 1 and 20.2. How many WADs do you have in your options? (DoomRPG Options -> Misc)

The bleeding system is indeed deadly, but bandages shouldn't be that rare even in the most unresourceful difficulty settings.
