It is impossible to resurrect a player whose head popped off. There is no longer a body to resurrect which can cause ugly crashes. As such resurrecting in this situation has to be disabled.

Just to be more specific, here it is from rh-log.txt:
April 16, 2006 (Changes by Graf Zahl)
- Fixed: A player that has no body (as in the head popped off in Heretic
and Hexen) must not be resurrected.

There is special handling, this was already addressed in some manner just recently.

When I fell to my death (extreme death) as the fighter, I typed in 'resurrect' in the console and rose up like normal, but my health skyrocketed to 999, I could turn but couldn't move, and I saw my exploded corpse on the ground. I think there must be some special handling regarding the fighter's head popping off during the extreme death sequence.
