This can be done fairly easily with an external addon; simply replace the graphic SELECTBO with any 32x32 graphic of your choice.

Small request here,
I am playing through Hexen and I'm noticing its kind of hard to see which item you are selecting. Perhaps it could stand out more with a green rectangle recticle or something.
I am using q and e keys to select items RMB to use and the standard wasd movement, which is probably not relevant,
