Because the structure used to read the "intermediate format" script headers defines the script number as WORD instead of SWORD, named scripts don't get detected when loading in that format. (Which is possible when using a compiler other than acc.)

I admit this may be the lowest priority bug in all of ZDoom's history, given the deprecation of not having the fake header, but it seems like an unnecessary limitation.

Meh, I suppose, though the likelihood of this ever being a problem in the real world is next to nil.

Would I be correct in assuming that you noticed this because you were going through the code to see how named scripts are implemented?

I noticed it because until recently, my compiler used the old system so I spent a few hours trying to figure out why my named scripts weren't working.
