ungayst has the nicest example of a ladder i've seen so far, dunno exactly how it's done tho, i think it's made with very little steps and wind blowing at the lightest level. take a look at it if you want, the map is pretty cool also.

You could always try Cyb's Helm's Deep.
This uses ThrustThingZ to get the player moving up the ladder.
It's also great fun, if something of a resourse hog.

You don't actually need Transfer_Heights, the water zone on it's own is enough. Yeah, there's the thrust-z command, which is what I've seen used most often, there's the very steep steps, possibly deep water... But I think the thrust-z thing is the best.
And that Helm's Deep thing... Holy crap... That's friggin insane!
Edit: Oh, and it looks like the LWM Hellspawn thingy is no longer up... Sucks, cause I wanted to take a look...
