Please, if you can include a wad demonstrating the problem, do so. Bug reports that include fully-constructed demos have a much better chance of being investigated in a timely manner than those that don't.

I recall seeing something about this once upon a time, but I couldn't find it. Any time I play any map (stock Doom 2, custom maps, etc.) the music does not loop for some reason. After the first time the background music plays, it stops. It appears only to affect MIDIs, as MP3/OGG/IT seem to loop just fine. Now I have only reported this issue recently, I remember noticing this on versions of GZDoom since 1.1.3 Beta?

No, Janitor is right. Some Midi files don't loop.

I started playing Strife again with ZDoom and the music in the first map didn't sound right. I checked, and the Midi Device was set to Fmod. So I changed it to Awe64G Midi Synth, since I had a Soundblaster card when I first played it. I restarted the game and the music went back to what I remembered. But it didn't loop. I got a "Midi Playback Failure" message. So I started testing Doom2 wads and got the same error... sometimes. Some Midis (actually .mus files) looped fine, others gave the error message. I can't find a reason for the inconsistency. Some .mus files were taken directly from other games. Others were edited versions, which means they were converted to .mid, edited, then converted back to .mus. I did this testing using a WinME system with an Awe64 Gold card.

I went back to my CDs of old stuff and started testing old versions of ZDoom I had. I found that with version 2.2.0.751 the music loops fine but with 2.2.0.784 the error pops up, so this has been going on for quite a while. I know this isn't a huge issue, but some .mus files sound much better using the Awe64 Device than with Fmod.

I have attached two wads. One has music that loops correctly under the Awe64G Midi Synth device and the other has music that doesn't loop and gives the error message. I tried to pick short songs.

I had to take out my Audigy so cannot test with Creative's synth, but musfail loops fine with Microsoft's GS synth. I have made this error message more descriptive for revision 2383, so would anybody with this problem please try it and report the error?

Yes, I just tried it with r2386 on my WinME computer with my Awe64 Gold card. The musfail.wad still gives an error (but more descriptive) when using the Awe64G device. I also just tested it using the Microsoft GS Synth device. Same error.

Midi Playback Failure: Memory Allocation Error.

I'd like the OP to respond. An out of memory error in ME may just be ME being ME, but what's the error for Windows 7?
